- In a small bowl, whisk together ketchup, brown sugar, vinegar, Worcestershire, smoked paprika, garlic powder, and cayenne until smooth. Set aside.
- Combine breadcrumbs and milk in a large bowl and let sit for 5 minutes. Add the beef, egg, garlic powder, onion powder, salt, and pepper. Mix gently until just combined.
- Preheat oven to 400°F (200°C). Line a baking sheet with parchment. Form the mixture into 1 1/2-inch meatballs (about 20) and place them on the sheet. Bake for 18 to 20 minutes, until browned and cooked through (160°F internal).
- Remove the pan, turn the broiler to high, and brush the meatballs generously with the glaze. Broil 2 to 3 minutes, watching closely, until the glaze is bubbly and sticky. Serve immediately.
Notes
If you like extra glaze for serving, double the sauce ingredients and set half aside before brushing the meatballs. The meatball mixture can be shaped and refrigerated up to 24 hours ahead; increase the bake time by 2 to 3 minutes if baking straight from the fridge.
